Adding a Consent Statement to a Survey

When you are designing surveys to collect personal information, it is important that you receive consent. If you are collecting reports from EU citizens, you are required by the General Data Protection Regulation (GDPR) to have a lawful basis for the data you collect, usually by receiving positive, opt-in consent.

To add terms of consent to your Survey

  1. Log in to your deployment
  2. In the main nav, go to Settings
  3. Within Settings select Surveys
  4. Click on the Survey you’d like to edit
  5. At the bottom of the survey “add field”
  6. Choose field type: Text Boxes
  7. Name your consent form. For example: “Terms of Service”
  8. Toggle “Show field description”
  9. Paste your terms and any links into the description field
  10. Choose “Add” and write what your users are consenting to
  11. Make this a required field
  12. Click “Add and Close”
